An exciting opportunity exists for the Aboriginal and Torres Strait Islander Hospital Liaison Officer to support and work collaboratively with multidisciplinary teams and primary care providers within the Sunshine Coast region to ensure culturally appropriate and safe health services contribute to the improved health and well-being of Aboriginal and Torres Strait Islander patients.
Sunshine Coast Hospital and Health Service (SCHHS) is in southeast Queensland and extends through the coastal and hinterland areas from Caloundra in the south to Gympie in the north.

**Key Responsibilities include**:
- Facilitate and maintain close liaison links with staff to enhance access to health services and practical social support services for Aboriginal and Torres Strait Islander patients and their families.
- Develop linkages with all relevant hospital wards, departments, and community services to facilitate admission, discharge planning and referral to follow-up services.
- Network and liaise between Aboriginal and Torres Strait Islander community organisations, public sector agencies, patients, their families and Aboriginal and Torres Strait Islander Programs within the Hospital and Health Service.
- Promote and facilitate the identification process for Aboriginal and Torres Strait Islander patients to staff on admission and assist in improving processes.
