**Your new company**
We are currently supporting one of our key government clients that operates an office based clinical support team in Perth CBD. This team is designed to support vulnerable people and their families who are experiencing mental health challenges. Working in an open plan office environment with a small clinical team, you will work alongside experienced mental health clinicians.

**Your new role**
In your new role, you will be handling a complex clinical caseload, providing support and guidance to other medical professionals when required. As a primary point of contact, you will be working with a variety of clients experiencing mental health challenges and coordinating their access to various services. This is a high volume case management role. Key responsibilities include:

- Act as a single point of contact for those in your caseload
- Developing risk management plans for at risk clients and escalating high risk profiles when needed
- Liaise with a wide variety of personal including internal stakeholders, service providers and community groups
- Providing support over the phone and virtually to clients
- Maintaining detailed and accurate case notes in line with requirements

**What you'll need to succeed**
In order to be successful in this role, you will need to have proven experience in a clinical case management environment. This position requires an experienced clinician who has managed a high-volume complex caseloads supporting vulnerable clients. Key requirements include:

- Must be an AASW Social Worker, AHPRA registered Psychologist, Mental Health Registered Nurse or Mental Health Occupational Therapist
- Proven high volume case management experience
- Ability to assist and support management with complex caseloads
- Effective written ability to complete case notes
- Significant experience performing triage and risk assessments
- Excellent interpersonal skills to develop relationships and liaise with a variety of personnel
- Current Police check, or willingness to obtain one
- Must be an Australian Citizen
