**About the role**

As a Support Coordinator, you will be responsible for assisting National Disability Insurance Scheme (NDIS) participants in activating, interpreting and implementing their NDIS Plans, with a strong focus on seeing participants move towards their NDIS goals. You will also be responsible for client growth in the Tweed region.

You will work closely with NDIS participants and their families/guardians to connect with individual services and supports outlined in their NDIS Plan, ensuring they get the most out of their funding and achieve outcomes.

Responsibilities include:

- Work in consultation with clients and their families to find the services and supports that match their needs and funding
- Coordinate services and supports, liaise with government agencies, obtain quotes and organise service agreements
- Develop, monitor, review and report on your client’s NDIS plans and share progress with key stakeholders
- Work within the funding allocated and regularly monitor and track billable coordination hours
- Building and maintaining relationships with our clients, their families, staff, peers, other services and stakeholders

**About you**

We’re looking for someone with:

- A strong understanding of the NDIS and its funding and policy protocols, standards and processes
- Qualifications in Social Work, Allied Health, Case Management or equivalent experience is desirable
- Highly developed communication, negotiation and problem-solving skills
- Strong interpersonal and relationship-building skills with internal and external stakeholders
- Current Australian drivers licence
