Key Role: Case Coordinator

This position requires a skilled professional to manage cases, develop support plans and provide individualized services to participants. The coordinator will work within a multi-disciplinary team delivering coaching and training through participant tailored methods.

The ideal candidate must possess assessment skills, case management skills, and the ability to coordinate multiple service providers to deliver client outcomes.

  • Relevant education or qualifications in social sciences, behavioural health or equivalent experience.
  • Experience working with diverse clients including those from CALD backgrounds and/or Aboriginal clients.
  • Ability to assess and support clients with complex needs.
  • Demonstrated report writing and case notes skills.
