Job Description

Clinical Care Coordinators play a vital role in delivering high-quality care and services to residents in residential aged care settings.

The Clinical Care Coordinator will coordinate the care team, ensuring compliance with regulatory requirements and facilitating collaboration with facility leaders.

This is a challenging and diverse role that requires strong communication skills, the ability to work independently, and a commitment to delivering exceptional patient care.

Key Responsibilities
  • Manage all clinical services aligned with organization's missions and policies.
  • Advocate for continuous improvement activities in clinical and lifestyle services.
  • Ensure accurate and timely completion of clinical documentation.
  • Coordinate and support scheduled audits and clinical indicators.
  • Ensure resident health records and clinical documentation are up-to-date.
Required Skills and Qualifications
  • Registered Nurse with current registration.
  • Current police check or willingness to obtain.
  • Current NDIS check or willingness to obtain.
  • IPC training completed or willingness to complete.
  • A commitment to work within organization's Mission, Vision and Values.
  • Demonstrated knowledge and understanding of aged care legislation.
  • Commitment to contemporary nursing practice.
  • Ability to support a multi-disciplinary team.
  • Effective communication skills, oral and written.
  • Demonstrated alignment with resident-focused care philosophy.
  • Knowledge of AN-ACC and Aged Care standards.
