Role Overview

Clinical Care Coordinator

This role oversees the coordination of care teams in collaboration with facility managers to ensure compliance with legislative and statutory requirements, providing high-quality care and lifestyle for residents.

  • Leading clinical services in accordance with organizational missions and policies
  • Implementing continuous improvement initiatives across all aspects of clinical and lifestyle services
  • Ensuring accurate and timely completion of clinical documentation
  • Coordinating scheduled audits and clinical indicators
  • Maintaining resident health records and clinical documentation up-to-date and relevant

About You

We seek a Registered Nurse with current registration with AHPRA and a commitment to working within our organization's Mission, Vision, and Values.

  • Current police check or willingness to obtain
  • Current NDIS check or willingness to obtain
  • IPC training completed or willing to complete
  • Demonstrated knowledge and understanding of governing legislation in aged care
  • Commitment to contemporary nursing practice in aged care
  • Ability to support a multi-disciplinary team
  • Effective communication skills, oral and written
  • Demonstrated alignment with resident-focused care philosophy supporting dignity, independence, and informed consultative decision-making
  • Knowledge of AN-ACC and Aged Care standards
