Key Responsibilities- Respond to referrals and conduct comprehensive assessments with young people to identify their needs and develop individualized case plans.
- Provide ongoing case management to clients, including coordination of referrals to relevant services and support networks.
- Develop and maintain relationships with internal and external stakeholders, including government agencies, non-government organizations, and community services.
- Collaborate with colleagues to ensure a comprehensive and coordinated approach to supporting young people.
- Conduct regular reviews of client progress and adjust case plans as needed.
- Maintain accurate and up-to-date records of client interactions and case management activities.
- Tertiary qualification in a relevant field, such as social work, youth work, or a related discipline.
- Experience working with young people who are homeless or at risk of homelessness, including those with complex needs.
- Strong communication and interpersonal skills, with the ability to build rapport with clients, colleagues, and external stakeholders.
- Ability to work independently and as part of a team, with a high level of autonomy and flexibility.
- Current NSW Driver's License and willingness to travel to remote areas.
- Senior First Aid Certificate or willingness to obtain.
