Home Care Team Lead

Job Summary:
Austcorp Executive Recruitment PTY Ltd is seeking a dedicated and skilled Care at Home Coordinator to lead and grow our Home Care team. In this role, you will report to the Care at Home Manager and be tasked with cultivating sustainable relationships with clients and their families, managing staff to ensure top-notch care delivery, and engaging in promotional activities to drive the brand growth.
Key Responsibilities:
- Provide exceptional, adaptable care to clients in alignment with Home Care Standards.
- Foster strong connections with clients and their families, ensuring their needs are met and exceeded.
- Establish and maintain professional networks with local stakeholders, promoting our services and building partnerships.
- Conduct client assessments, care planning, and case management in collaboration with clients, ensuring personalized and effective care.
- Lead, manage, and support staff in delivering high-quality, client-focused services, promoting a positive and productive work environment.
- Participate in quality improvement initiatives, driving excellence and innovation in our services.
- Achieve budget targets while driving excellent outcomes for our clients, ensuring the financial sustainability of our services.
Requirements:
- A recognized qualification in Aged Care or a related field, demonstrating expertise and knowledge in the sector.
- At least 3 years of experience in coordinating aged care, community services, health, or social services, with a proven track record of success.
- In-depth knowledge of Home Care legislation, policies, and procedures, ensuring compliance and best practice.
- Strong planning, organizational, and management abilities, with a focus on delivering high-quality services.
- Exceptional customer service skills, with a passion for delivering personalized and effective care.
- Proven experience in leading, developing, and managing teams, promoting a positive and productive work environment.
- Strong problem-solving skills and attention to detail, with a focus on delivering high-quality services.
- Budget management experience, with a focus on achieving financial sustainability.
- Comprehensive understanding of service coordination and case management principles, including client-centered assessment, planning, delivery, and evaluation.
- Proficient in Microsoft Office and other relevant software, with a focus on efficient and effective service delivery.
- A current driver's license, with a focus on safe and reliable transportation.
