About the Role

We are seeking a highly skilled and experienced Care Manager to join our team at Right at Home, a leading provider of home care services in Australia.

As a Care Manager, you will be responsible for overseeing the delivery of high-quality care to our clients, ensuring their needs are met and exceeded. You will work closely with our team to develop and implement care plans, manage cases, and recruit and train caregivers.

Key Responsibilities
  • Care Assessment and Planning: Conduct thorough assessments of clients' needs and develop personalized care plans to ensure their safety and well-being.
  • Caregiver Management: Recruit, train, and manage a team of caregivers to provide high-quality care to our clients.
  • Operational Management: Oversee the day-to-day operations of our home care services, ensuring compliance with regulatory requirements and company policies.
  • Quality Assurance: Monitor and evaluate the quality of care provided to our clients, identifying areas for improvement and implementing changes as needed.
Requirements
  • Qualifications: A qualified Registered Nurse with a strong understanding of aged care principles and practices.
  • Experience: Proven experience in a care management role, with a strong track record of delivering high-quality care and managing teams.
  • Skills: Excellent communication and interpersonal skills, with the ability to work effectively with clients, caregivers, and other stakeholders.
  • Leadership: Strong leadership skills, with the ability to motivate and inspire a team to deliver exceptional results.
