About the Role

We are seeking a Team Leader to join our team, providing high-quality care to our residents. This is an integral role in ensuring individual needs, wishes and goals of each resident are met.

The Team Leader will support the Clinical Care Coordinator and Registered Nurse to supervise and coordinate care across the home while ensuring care provision is holistic and person-centred.

Key Responsibilities
  • Provide social and emotional support to all residents.
  • Support the coordination of care needs of residents including medication management, nutrition and hydration, and pressure care management.
  • Lead/supervise care on the shift within designated wing or area.
  • Perform medicine related tasks in accordance with scope of qualification, legislation, regulation and policies.

  • An effective listener with ability to communicate at all levels both orally and in writing.
  • Ability to understand and use information technology and programs.
  • Current National police check and Working knowledge of WH&S and Infection Control.
  • A desire to make a positive difference in the community.
