Compassionate Home Care Specialist

We are seeking a dedicated and compassionate Home Care Coordinator to join our team. As a key link between clients, their families, and our care team, you will ensure the highest quality home care services by managing schedules, coordinating care plans, and providing support.
About this role:
- Match clients with caregivers based on needs and preferences
- Schedule and coordinate in-home care services
- Communicate with families, caregivers, and healthcare professionals
- Monitor and update client care plans
- Ensure compliance with relevant standards and best practices
- Provide exceptional customer service and client support
What we're looking for:
- 2 years experience in aged care/disability care and knowledge of relevant standards preferred
- Strong organizational and communication skills
- Ability to multitask and work in a fast-paced environment
- A caring and compassionate attitude
- Proficiency with scheduling software and basic computer skills
Requirements:
- Certificate III/IV in Individual Support (Ageing and Disability) or equivalent
