Program Respite Coordinator

The program aims to enhance the wellbeing of family carers and people living with dementia by offering tailored respite experiences.

About the Role

We seek an experienced program coordinator to support this initiative. The successful candidate will develop person-centred care plans, work closely with clients and carers to deliver flexible respite options, and improve outcomes for people with dementia who wish to remain living well at home for longer.

Key Responsibilities
  • Support the manager in implementing the program with clients and carers.
  • Engage with participants to provide timely, person-centred dementia respite support.
  • Identify and coordinate informal and formal respite options tailored to individual needs.
  • Use validated tools to support self-care and wellbeing reflection.
  • Contribute to the development of a respite model based on pilot outcomes.
  • Deliver individual and group education on respite and dementia care.
Requirements
  • COVID-19 vaccination per policy requirements.
  • Ability to obtain a National Police Clearance (within 6-months validity).
  • Successful completion of pre-employment form and reference checks.
  • A formal qualification in a related field, e.g., allied health, nursing.
  • Current APHRA Registration without restrictions, or equivalent.
  • Western Australian C Class Driver's License.
  • Experience supporting people living with dementia.

This is a part-time fixed-term role working 75 hours per fortnight, with flexibility on which weekdays are worked. Hours are 8:30am to 4:30pm, based at our Head Office.
