Care Navigator

We are seeking a part-time Care Navigator to be based at Ngala, Kensington.
As a Care Navigator, you will work with a child-centred, strength-based, and collaborative approach to provide support to new parents and their families.
Key Responsibilities- Identify state-wide PIMH support services and referral pathways.
- Build productive relationships and stakeholder engagement with key individuals and groups.
- Connect and care-specific tasks, including intake, screening, and triaging and allocation of families to local services.
- Ensure the effective coordination and integration of services, timely, accurate, and quality record keeping, and project monitoring.
You will possess:
- Bachelor or Diploma in Social Work, Nursing, or Psychology.
- Current Registration with the Australian Health Practitioners Regulation Agency (AHPRA); Nursing or eligibility for membership with the peak professional association (Social Workers).
- Knowledge and understanding of mental health issues in antenatal, birth, and the postnatal period.
- Demonstrated experience of collaborative person-centred care and treatment planning and working with families.
- Demonstrated ability to work in a multidisciplinary team and liaise with other health professionals.
- Salary Packaging up to $9,010 to extend take-home pay.
- 17.5% leave loading on 4 weeks Annual Leave.
