We are seeking a part-time Care Navigator to join our team at Ngala, Kensington. As a Care Navigator, you will play a critical role in supporting new and expecting parents with mental health navigation services.
Key Responsibilities- Identify state-wide PIMH support services and referral pathways.
- Build productive relationships and stakeholder engagement with key individuals and groups.
- Connect and care specific tasks, including intake, screening, and triaging and allocation of families to local services.
- Ensure the effective co-ordination and integration of services, timely, accurate and quality record keeping, and project monitoring.
You will possess:
- Bachelor or Diploma in Social Work, Nursing or Psychology.
- Current Registration with the Australian Health Practitioners Regulation Agency (AHPRA); Nursing or eligibility for membership with the peak professional association (Social Workers).
- Knowledge and understanding of mental health issues in antenatal, birth and the postnatal period.
- Demonstrated experience of collaborative person-centred care and treatment planning and working with families.
- Ability to undertake a comprehensive perinatal-specific biopsychosocial assessment and use this to inform referral pathways and plan treatment as required in the perinatal context.
- Salary Packaging up to $9,010 to extend take home pay.
- 17.5% leave loading on 4 weeks Annual Leave.
