Independent Living Specialist

Our Home Support Assessment team is looking for a skilled professional to join them in the Loddon-Malee Region. As a dedicated Support Assessor, you will conduct face-to-face home support assessments with older Australians to enable access to the care they need to remain living independently in their chosen community.
You will work closely with clients to develop support plans with individualised goals, matching and referring them to services that can assist them to achieve optimal wellness and healthy ageing.

To be successful in this role, you'll need:
- A keenness to empower individuals and support their independence
- Competent computer skills, as you'll be inputting information into a laptop
- The ability to work autonomously, conducting assessments in-person at clients' homes
- Strong communication skills to build rapport and trust with clients
