Carbal Medical Services is seeking a skilled Service Delivery Manager to lead the development and implementation of programs tailored to meet the unique needs of our ageing communities. The successful candidate will be responsible for managing and leading aged care funded programs, ensuring compliance with relevant standards and funding guidelines.



Main Responsibilities


  • Implement, manage and lead aged care funded programs under the quality management framework and Aged Care Quality Standards.
  • Recognise the differences in care needs for Indigenous vs non-Indigenous clients and the complexities involved while ensuring the delivery of client-centred services.
  • Oversee the daily operations of service delivery ensuring compliance with the relevant standards and funding guidelines.
  • Lead and mentor a dedicated team of care partners, optimising individual health outcomes of clients and effective management of package budgets.
  • Understand the financial resources required and collaborate with the finance team to ensure sustainability and efficiency of services.
  • Engage and collaborate with diverse stakeholders including ACAT and RAS to ensure seamless service integration and coordinated care for clients.


Requirements


  • Completed tertiary qualification and/or relevant experience in a health-related field.
  • Proven leadership and operational capabilities, preferably within the Aged Care industry.
  • Knowledge of the National Safety and Quality and Aged Care Quality Standards and the aged care funding principles through My Aged Care including but not limited to CHSP, HCP.
  • Commitment to quality improvement and culturally responsive service delivery.
  • Open C class drivers licence, blue card and willingness to undergo a Police Check.
  • APHRA Registration.
  • A demonstrated understanding of the health and social issues found in our Indigenous Communities.
  • Previous experience working in the Not-For-Profit sector.
