About the Role

We are seeking a highly skilled and compassionate Clinical Case Manager to join our team at integratedliving Australia Ltd. As a Clinical Case Manager, you will play a vital role in empowering older adults to thrive by providing care management and coordination services.

Key Responsibilities
  • Provide care management and coordination services to older adults in their homes
  • Develop and implement personalized care plans to meet the unique needs of each client
  • Collaborate with healthcare professionals, family members, and caregivers to ensure comprehensive care
  • Conduct regular assessments and evaluations to monitor client progress and adjust care plans as needed
  • Provide education and support to clients, caregivers, and family members on health and wellbeing topics
  • Facilitate group workshops and support sessions to promote healthy lifestyle and self-management skills
  • Coordinate care with multiple disciplines, including GPs, specialists, and allied health professionals
Requirements
  • Degree qualification in nursing or allied health
  • Registration with relevant professional body or Australian Health Practitioner Regulation Agency (AHPRA)
  • 3+ years of clinical or health management experience in nursing or allied health
  • Experience working in a health or community aged care setting
  • Ongoing commitment to continued professional development and education
  • Demonstrated understanding of community aged care guidelines and the principles of primary health care and health promotion
  • Demonstrated understanding of evidence-based care
  • Ability to conduct clinical case management and health assessments
  • Strong interpersonal skills, problem-solving, analytical, and reporting skills
  • Ability to work as part of a wider team and autonomously in a remote model of working
  • Excellent time management skills to prioritize client needs and manage a busy schedule
  • Strong IT and administration skills and demonstrated competence in delivering services via virtual health technologies
