Job Summary

We are seeking an experienced Aged Care Case Manager to join our team in Mawson. The successful candidate will be responsible for providing case management services to clients, promoting our services and building relationships with community members.

Key Responsibilities:
  • Assess client needs and develop individualised care plans;
  • Provide case management services to clients, including coordination of care and support;
  • Promote our services to build client numbers and engage with the local community;
  • Work collaboratively with our team of Support workers to provide high-quality care and support services;
Requirements

To be successful in this role, you will need:

  • Demonstrated experience in client assessment and case management in a community setting, preferably in aged care;
  • Sound understanding of relevant legislation, including the Single Funder Model;
  • Ability to work collaboratively within a small team environment;
  • Demonstrated knowledge of community resources and professional services available to clients;
  • Ability to work effectively with culturally diverse groups;
  • Excellent communication skills, both written and verbal;
  • Proficient in the use of computer software, including Word, Excel, Internet and email;
Preferred Qualifications:
  • Experience using Client Management Systems, such as Techone or Alayacare;
  • Understanding of the ageing process and its cultural impact;
  • Ability to speak a community language/s.
