Community Care Team Member

We are seeking applications from passionate healthcare professionals to join our team in delivering person-centred care in the community.

This role involves providing high-quality clinical services to clients, ensuring a safe environment and empowering individuals to live fully through client-focused activities.

Location: Morayfield, Woodford & Surrounding Areas. Employment type: Permanent Part Time.

Remuneration: Competitive hourly rate + 12% Super + Salary Packaging options + Allowances & Travel Reimbursement + Employee discounts.

Responsibilities:
  • Assess, plan and implement client care within your scope of practice
  • Consider physical, social and psychological requirements of clients when developing care plans
  • Ensure clinical quality compliance standards are met through effective collaboration with multidisciplinary teams

Most days involve on-the-road community visits and scheduled client visits, requiring excellent communication and time management skills.

Role Requirements:
  • Full, unrestricted registration as a Registered Nurse and unlimited work entitlements
  • General nursing experience, post-registration
  • Current Australian Driver's Licence (Green Provisional minimum) – Essential
  • Roadworthy and insured vehicle to use for work – Essential
