Job Title: Community Nursing Specialist

We are seeking a skilled and dedicated Community Nursing Specialist to join our team of healthcare professionals.

The successful candidate will provide high-quality nursing care to patients in their own homes, working collaboratively with other healthcare professionals to deliver seamless and holistic care.

About the Role

As a Community Nursing Specialist, you will be responsible for providing skilled nursing care to patients in Rainworth and surrounding villages. You will work closely with patients and their families to develop and implement personalized care plans that meet their unique needs and goals.

You will also facilitate the prevention of unnecessary hospital admission or readmission by reducing the effects of ill health and disability through proactive interventions and support.

Key Responsibilities:
  • Conduct comprehensive assessments of patients' physical, emotional, and social needs.
  • Develop and implement personalized care plans that address patients' unique needs and goals.
  • Provide high-quality nursing care to patients in their own homes, including wound management, medication administration, and vital sign monitoring.
Required Skills and Qualifications:
  • Professional qualification in nursing (RGN) and current registration with the NMC.
  • Degree-level education or equivalent experience.
  • Excellent communication and interpersonal skills.
