Key Responsibilities:

  • Provide personalized care and support to clients in their homes, promoting safe and independent living.
  • Work closely with clients, families, and advocates to understand their needs and preferences.
  • Communicate effectively with clients, families, and colleagues to ensure seamless care and support.
  • Monitor and report changes in clients' well-being, and escalate any concerns to a Coordinator or On Call.

Requirements:

  • Previous experience in community home care with older people.
  • Proven ability to communicate effectively with clients and stakeholders.
  • Essential minimum qualification: Certificate III in Individual Support or Community Services.
