About the RoleThe Home Care Package Coordinator plays a vital role in ensuring a customer-focused culture, providing compassionate support and care to clients and their families while delivering exceptional service.
Key Responsibilities- Oversee and review client care plans, ensuring quality standards are met through active participation in clinical audits.
- Foster holistic client care by understanding the roles of various health care professionals, enabling effective cross-referrals and collaborative multidisciplinary teamwork.
- Coordinate staff training programs to enhance workforce skills and ensure up-to-date care practices.
- Address and resolve client concerns and service delivery issues through thorough investigation and responsive action.
- Proven experience in a supervisory role.
- Strong background in planning, developing, implementing, and evaluating programs.
- Expertise in budget management and preparing monthly reports.
- Demonstrated ability to meet service requirements outlined in formal contracts and agreements.
- Experience collaborating with government, non-government, and community organisations to deliver client services.
- Previous experience in a health-related field.
- ACAT assessment experience is highly desirable.
