We are seeking a highly skilled and compassionate Service Coordination Specialist to join our team at Catholic Healthcare. This is an exciting opportunity to make a meaningful difference in the lives of older Australians.
About the Role:- As a Service Coordination Specialist, you will play a critical role in assessing the complex needs of clients and referring them to appropriate aged care services under the Aged Care Act.
- You will work collaboratively with multidisciplinary teams, clients, and their families to ensure they receive the support they need to maintain their independence and quality of life.
- Your primary focus will be on conducting accurate assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services.
- Collaborate with clients, families, and multidisciplinary teams to identify and meet the complex needs of clients.

- Review support plans to meet changing client needs and preferences.
- Work effectively as part of a broader team to deliver key performance indicators within identified timeframes.
- Maintain currency of knowledge relating to aged care services, both funded and non-funded.
- Tertiary qualification in a health-related discipline directly related to Health, Aged Care, or a related specialist area.
- Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA).
- Australian Driver's license and access to a registered, insured vehicle.
- Minimum one year demonstrated experience in Australia or overseas delivering services in Aged Care settings and/or to aged persons, including those living with dementia.
- A competitive salary package, estimated to be around $54.47 per hour plus superannuation, reflecting your expertise and commitment to the role.
