Job Opportunity: Care Partner

Southern Cross Care QLD is seeking a proactive and self-directed full-time Care Partner to join their Home Care team on the Sunshine Coast.

This dynamic role involves managing a caseload of clients receiving Home Care Packages (HCP) and Commonwealth Home Support Program (CHSP) services, driving towards exceptional care outcomes.

  • Conducting comprehensive needs assessments for each client in a caseload
  • Developing personalized care plans in collaboration with clients and their families
  • Coordinating the delivery of services such as personal care, meal preparation, cleaning, and transportation
  • Monitoring the implementation of care plans and making adjustments as needed
  • Ensuring CHSP services are provided in accordance with guidelines
  • Advocating for clients' rights and preferences
  • Conducting regular reviews and reassessments to ensure that clients' needs are being met effectively
  • Providing information and support to clients and their families regarding available services and resources
  • Liaising with healthcare professionals, service providers, and community organizations to ensure coordinated care

The ideal candidate will have experience in a similar role within Aged Care or Disability Services and demonstrated knowledge and experience in delivering services under Home Care Package Program and Commonwealth Home Support Program.
