The role of a Social Worker in a hospital setting involves providing medical-focused social work services to patients with complex needs.

Collaboration with multidisciplinary teams is essential for discharging patients with complex social issues. This includes participation in the Aged Care Assessment Service (ACAS) assessment process and managing related administrative tasks.

Developing and implementing discharge plans that align with patient assessments, including engagement with carers and families, is also a critical responsibility. Additionally, coordinating community resources to ensure suitable discharge options is key to successful patient outcomes.

Requirements
  • Bachelor of Social Work (minimum).
  • Proven hospital experience as a Social Worker.
  • Eligible for registration with the Australian Association of Social Workers (AASW).
  • Strong knowledge of the Aged Care sector and General Medical social work.
  • Ability to provide evidence-based practice in line with AASW guidelines.
  • A valid National Police Clearance and Working with Children Clearance.
  • Familiarity with issues affecting Aboriginal and Torres Strait Islander peoples.
