Job Title: Community Interlink Care Manager

The Community Interlink Care Manager will provide guidance and support to vulnerable community members in the Seymour and surrounding region to help them achieve their goals and maintain independence.

The role involves utilising care plans, assessments, and risk mitigation strategies to ensure the safety of consumers. It also requires sourcing available providers to engage services and products that promote independence and goal attainment.

This is an opportunity to work with a supportive team who will help upskill and build knowledge in both the Aged Care and Disability Support Sectors.

The successful candidate will be based out of Seymour, Mansfield, or Benalla and will have the opportunity to service consumers in the region.

Main Responsibilities:

  • Provide guidance and support to vulnerable community members
  • Utilise care plans, assessments, and risk mitigation strategies to ensure safety
  • Souce available providers and support the engagement of services and products
  • Work with a supportive team to upskill and build knowledge
