Aboriginal Child and Maternal Care Specialist
We are seeking a dedicated professional to join our team as an Aboriginal Child and Maternal Care Specialist. This role will involve providing high-quality care coordination services to support continuity of care for vulnerable populations.
About the Role:
The successful candidate will be responsible for undertaking care coordination for a small caseload, identifying and accessing appropriate referral pathways based on individual needs and goals of care, and liaising with families, carers, health practitioners, and other stakeholders involved in provision of services.
Duties and Responsibilities:
- Undertake care coordination for a small caseload across Southern Metropolitan Adelaide.
- Identify and access appropriate referral pathways based on individual needs and goals of care.
- Liaise with families, carers, health practitioners, and other stakeholders involved in provision of services.
- Contribute to improved cultural competence across the organization and develop enhanced pathways and links with community health and wellbeing services.
About You:
To be successful in this role, you will need to demonstrate a strong ability to liaise, consult, and engage with the Aboriginal community and individuals in a culturally sensitive manner. You should also have excellent interpersonal, written, and verbal communication skills, including the ability to provide analytical commentaries and consultancy services to staff at all levels.
