Job Description

This is an exciting opportunity for a dedicated and compassionate physiotherapist to work with our multidisciplinary team, providing individualised physiotherapy services to clients in the community. As a Senior Rehabilitation Physiotherapist, you will have the autonomy to structure your day and work independently, while also being part of a collaborative team that promotes continuous quality improvement.

Your primary responsibility will be to deliver high-quality physiotherapy services to clients in their homes, clinics, or other settings, focusing on rehabilitation and restorative care. You will work closely with our experienced team to provide comprehensive care, develop treatment plans, and monitor client progress.

Key Responsibilities:
  • Deliver person-centred physiotherapy services to clients in various settings, including home visits and group activities;
  • Conduct assessments, develop treatment plans, and implement interventions to improve client outcomes;
  • Collaborate with our multidisciplinary team to ensure seamless care coordination and communication;
  • Document client progress, maintain accurate records, and participate in quality improvement initiatives;
  • Contribute to the development and implementation of new programs and services.
Requirements

To be successful in this role, you must have:

  • A tertiary qualification in Physiotherapy and current AHPRA registration;
  • Practical experience in a rehabilitation or aged care setting, preferably in a community-based environment;
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals;
  • Strong organisational and time management skills, with the ability to prioritise tasks and manage multiple projects simultaneously;
  • A reliable vehicle, valid driver's license, and safe driving record, as regular travel between client homes and HammondCare locations is required.
