Occupational Therapist Job Overview

This is a challenging and rewarding opportunity for an Occupational Therapist to join our organization.

The successful candidate will be responsible for providing care planning and advance care planning for patients with late-stage chronic and complex health conditions.

You will conduct palliative care needs assessments and develop collaborative treatment care plans, working closely with multidisciplinary teams.

Key Responsibilities:

  • Care planning and advance care planning for patients with chronic and complex health conditions.
  • Palliative care needs assessments and treatment care plans.
  • Referrals to relevant internal and external health and community support providers.
  • Education and support to other clinicians.

Requirements:

  • Current Registration as an Occupational Therapist with AHPRA or eligibility for accreditation as an Accredited Exercise Physiologist with ESSA.
  • Extensive knowledge and experience with consumers who have chronic and complex illnesses.
  • Experience in care planning and case management of community-based patients.
  • Understanding of specialist palliative care and community health services.
  • High level interpersonal and communication skills.
  • Clinical leadership capabilities.
