About the Role

We are seeking an experienced Coordinator to join our team based in our Bunbury office. As a Client Liaison Coordinator, you will manage a caseload of Aged Care or NDIS funded clients, connecting them to internal and external services, guiding them through their Aged Care or NDIS journey, and assisting them in achieving their defined and funded goals.

Key Responsibilities
  • Develop and Implement Service Agreements
  • Address Feedback and Assess Service Referrals
  • Monitor Client Budgets and Costings
  • Conduct In-Home Client Visits
  • Ensure Compliance with Aged Care and NDIS Standards
Requirements
  • Experience in Disability Services or Aged Care Services
  • Excellent Interpersonal and Communication Skills
  • Genuine Desire to Make a Difference
  • NDIS Worker Screening Check
  • Australian 'C' Class Drivers Licence
  • Exceptional Time Management and Organisational Skills
