Career Opportunity: Clinical Case Manager

Your Role:

  • You'll guide clients through a transformative 8-week journey in our Short-Term Restorative Care (STRC) program, empowering them to reclaim their independence and confidence.

Your Day-to-Day Might Include:

  • Co-designing personalised care plans that focus on rehabilitation and wellness with clients at home.
  • Running engaging group sessions that promote healthy living and self-management.
  • Collaborating with healthcare professionals to deliver holistic care.
  • Using digital tools to provide virtual coaching and support.
  • Building strong local networks and promoting the STRC program in your community.

What You'll Bring to the Table:

  • A degree in Nursing or Allied Health, with current registration.
  • At least 3 years' experience in clinical care, health coaching, or care coordination – especially in aged or community care.
  • A passion for health promotion, preventative care, and client empowerment.
  • To confidently navigate digital platforms for virtual care delivery, accurate documentation, and communication – high-level computer skills are essential.
  • A valid driver's license and a love for getting out into the community.
