Family Preservation Case Manager Job Opportunity

We are seeking an experienced and dedicated Family Preservation Case Manager to join our team in a challenging and rewarding role.

Key Responsibilities:

  • Work intensively with a caseload of families to support them in keeping their children safe at home through evidence-based practices and interventions.
  • Conduct comprehensive assessments and develop, implement, and review clinical therapeutic treatments and interventions tailored to each family's unique needs.
  • Identify strengths and areas of risk for each family and collaborate with other professionals to provide wrap-around support.
  • Develop and maintain effective relationships with clients, colleagues, and community stakeholders to ensure seamless service delivery.

Requirements:

  • Qualifications in Community Services or a relevant field with a focus on working with vulnerable populations.
  • Significant experience in case management, preferably with Aboriginal families, including experience in home-based services and community outreach.
  • Ability to work effectively in a culturally sensitive environment, demonstrating cultural competence and humility.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with multidisciplinary teams.
