About the Role:

The Aboriginal Care Management Worker provides service brokerage and individualized supports to clients and their families, focusing on increasing social and emotional wellbeing.

This role works with individuals in community and residential programs addressing challenges such as alcohol and other drugs (AOD), chronic disease, mental health issues, domestic and family violence, housing, and other health and social concerns.

  • Requires being of Australian Aboriginal descent, identifying and being accepted by the local Aboriginal community.
  • Must have a current Certificate IV in Alcohol and Other Drugs or related field like Mental Health, Aboriginal Health, Community Services, Allied Health Assistance, or Social Care.
  • Demonstrated knowledge and understanding of the social and emotional needs and issues affecting Aboriginal people.
  • Proven experience working with Aboriginal clients and their families, managing a caseload effectively.
  • Able to communicate effectively with stakeholders, verbally and in writing.
  • Must hold a valid NT Drivers Licence.

  2. Developing individualized support plans to address these needs.
  3. Maintaining effective communication with service stakeholders.
  4. Collaborating with colleagues to ensure comprehensive support services.
