About our organization:

We are a dedicated provider of disability and aged-care services registered with the NDIS.

We offer a range of solutions including supported independent living daily living supports therapeutic and behavioural support plan management community engagement and nursing care tailored to individuals unique goals and needs.

Our team is committed to fostering independence and community connection in nurturing environments Key Responsibilities:

Provide person-centred care and supervision to adults with disabilities and mental health conditions e.g. schizophrenia depression PTSD dementia Conduct health and wellbeing assessments and monitor changes Collaborate with families allied health professionals and community agencies Coordinate external referrals and support plan implementation

Qualifications & Experience: Hold a Diploma or higher in Disability Community Services Social Work or similar 1–2 years experience in Supported Independent Living or residential mental health/disability care Have experience supporting individuals with complex mental health conditions Possess First Aid & CPR certification NDIS Worker Screening Blue Card & Working with Children Check Valid Australian driver's licence and reliable vehicle Completed NDIS Worker Orientation Module

Skills & Attributes: Excellent communication and interpersonal skills Proactive reliable and passionate about disability support Team-oriented with ability to work independently Strong organisational skills and attention to detail Proficient in Microsoft Office Excel Teams PowerPoint Committed to person-centred trauma-informed and culturally responsive care
