Job Title: Dedicated Community Care Nurse

Job Description

We are seeking a skilled Enrolled Nurse to provide high-quality care to our clients in the community. As a Community Care Nurse, you will be responsible for delivering wound and Catheter care services to individuals in their own homes or in residential settings.

As part of our team, you will have the opportunity to work independently while being supported by the clinical direction of a Registered Nurse. You will have flexibility in your work schedule, with the option to work 5 days per week from Monday to Friday.

The role requires excellent assessment, decision-making, and problem-solving skills to ensure the delivery of optimal patient care. With a strong focus on teamwork and collaboration, you will work closely with other healthcare professionals to achieve positive health outcomes for our clients.

Key Responsibilities:
  • Provide wound and Catheter care services to clients in their own homes or in residential settings
  • Work independently under the clinical direction of a Registered Nurse
  • Develop and implement individualized care plans that meet the unique needs of each client
  • Communicate effectively with clients, families, and healthcare teams to ensure seamless care coordination
  • Conduct regular assessments and evaluations to monitor client progress and adjust care plans accordingly
Required Skills and Qualifications

To be successful in this role, you will need:

  • Current Registration with the AHPRA as an Enrolled Nurse
  • At least 2 years of post-grad experience in an Enrolled Nurse role
  • Exceptional assessment, decision-making, and problem-solving skills
  • Excellent communication and interpersonal skills
  • A current driver's license
