About the role:
This temporary part-time position involves developing and implementing transition of care services for cardiology patients at a major hospital.
The ideal candidate will deliver advanced pharmacy clinical services, provide leadership and direction to multidisciplinary teams, and foster a collaborative work environment.
Key Accountabilities:
- Deliver specialist pharmacy services to cardiology patients with high independence.
- Develop and implement pharmacy practices, procedures, and protocols.
- Offer clinical advice and direction to healthcare teams for effective service delivery and quality outcomes.
- Collaborate with interprofessional teams to achieve departmental objectives.
