Family Based Care Case Manager

We are seeking a dedicated Family Based Care Case Manager to collaborate closely with children, young people, their families and carers.

  • Key responsibilities include creating and implementing case plans that strive towards achieving permanency goals of those we support.

As a valued member of our team, you will build and maintain strong, functional relationships with both the children and their families, ensuring that every voice is heard and every need is met.

Requirements:
  • Relevant bachelor's degree such as Social Work, Social Welfare, Psychology or similar qualification is desirable but not essential for Aboriginal and/or Torres Strait Islander applicants with relevant experience.
  • Previous case management experience within an Out of Home Care (OOHC) /Child Protection setting.
  • Experience supporting Children & Young People with disability, who have experienced backgrounds of trauma, abuse and/or neglect.
  • Working understanding of the NSW Permanency Support Program.
  • Understanding of trauma and its effects on children and young people.

To succeed in this role, you must be passionate about supporting vulnerable children and young people and their families.
