We are seeking a dedicated and compassionate Support Services Coordinator to join our team. This is an exciting opportunity for a skilled professional who is passionate about making a difference in the lives of individuals with disabilities.
About the RoleThe Support Services Coordinator will be responsible for providing support, information, and advice to clients on emotional, financial, recreational, health, housing, and other social welfare matters. You will evaluate and coordinate the services of welfare and community service agencies to ensure that clients receive the best possible care.
You will work closely with clients to identify their needs and develop personalized support plans. Your role will involve interviewing clients, assessing their difficulties, and planning educational and training programs to empower them to reach their full potential.
Key Responsibilities- Conduct interviews and assessments to identify client needs and goals.
- Develop and implement support plans tailored to individual client needs.
- Collaborate with community groups, welfare agencies, government bodies, and private businesses to secure resources and services for clients.
- Provide education and training to clients and their families to promote independence and self-sufficiency.
- Monitor client progress and report back to relevant authorities as necessary.
- Strong experience and knowledge in disability, mental health, and/or positive behaviour support.
- Demonstrated ability to work effectively with clients from diverse backgrounds and with varying levels of need.
- Certificate III Individual Support (Disability) or equivalent is desired.
- 2 years' work experience in disability service area or similar/related area.
- Possession of a current national Police Check with no disclosable outcomes.
- Mandatory COVID-19 vaccination with a TGA approved vaccine and ability to provide proof of vaccination status before being appointed to the role.
