**Job Description:**

We are seeking a skilled Supports Coordinator to join our team. The ideal candidate will work directly with clients and families, coordinating the right supports, building community connections, and ensuring services meet individual needs.

The role involves:

  • Coordinating supports and life transitions
  • Supporting employment and community access
  • Building daily living and life skills
  • Providing early intervention where needed
  • Helping with independent travel and transport
  • Working within guidelines to ensure the best outcomes for our clients

**Required Skills and Qualifications:**

Essential qualifications include:

  • Qualifications or experience in Disability, Social Work, Education, or related fields
  • Experience supporting people with disabilities (adults and/or children)
  • Understanding of person-centred and strengths-based practice
  • Strong written, verbal, and interpersonal communication skills
  • Ability to work independently, manage time, and meet deadlines
  • Confident working with budgets and reporting requirements
  • Skilled in building relationships and solving problems
  • Able to work respectfully with people from all backgrounds
  • Knowledge of relevant legislation and policies
