Job Description

We are seeking a dedicated and compassionate individual to support consumers in achieving their goals and living independently within the community.

  • The successful candidate will work closely with consumers, utilizing a strength-based approach to promote independence and autonomy.
  • In this role, you will assist in the delivery of reablement-focused home support plans by providing learning, guidance, and direction to consumers and their carers.
  • You will also support consumers with daily living tasks while promoting independence through the use of tools and initiatives outlined in the reablement or restorative focused support plan.

  • Certificate III in Aged, Disability, Community or relevant discipline is required.
  • Experience working in an aged, health, or community setting is essential.
  • A current motor vehicle driver's license is also necessary.
