The Role

We are seeking a Part Time experienced Chronic Disease Management (CDM) Nurse to join our team. As a CDM Nurse, you will support GPs and their patients to identify and manage their chronic conditions, promoting preventative healthcare and keeping patients out of hospital.

Key Responsibilities

  • Prepare Care Plans and Team Care Arrangements to help patients manage their disease/s
  • Empower patients to increase ownership over their health
  • Coach patients on embedding health behaviour change principles
  • Enable patients to access ongoing care from a multi-disciplinary team
  • Assist in early identification of chronic disease by conducting health assessments
  • Work with GPs to promote preventative healthcare throughout the practice
  • Increase the number of patients who receive a care plan for the first time
  • Ensure patient's care plans are reviewed regularly
  • Manage administrative tasks relating to carrying out CDM appointments

Requirements

  • AHPRA registered Enrolled or Registered Nurse
  • Proven prior experience in Chronic Disease Management and health assessments
  • Knowledge of Health Assessment is highly regarded
  • Strong organisational and time management skills
  • Outstanding communication and customer service skills
  • A passion for people and a personable approach
