About the Role: We are seeking a skilled and compassionate Direct Care Worker to join our team at Northern Grampians Shire Council. As a key member of our community care team, you will play a vital role in supporting clients to live independently by providing personal care, domestic, and social support.

Your Key Responsibilities:

  • Support clients to achieve and maintain their wellbeing and independence through individualised care plans and services.
  • Provide a range of home and community care services, including assistance with domestic tasks, personal care, and respite care.
  • Communicate effectively with clients, carers, and other stakeholders to ensure seamless delivery of care services.
  • Maintain accurate records and reports, as required.
  • Participate in continuous improvement and professional development processes to enhance your skills and knowledge.

About You: To be successful in this role, you will have:

  • A minimum Certificate III in Individual Support (or equivalent) and competencies in Level 2 First Aid and CPR.
  • Experience in providing general household duties, respite care, and personal care tasks.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups.
  • An understanding of the philosophies of healthy ageing and the Active Service Model.
