We are seeking an experienced Senior Care Professional to join our team. As a Senior Care Professional, you will conduct initial client assessments and develop personalized care plans that cater to their unique needs.
- Conduct thorough initial screening assessments and develop care plans for personal self-care, general house cleaning, social support, respite, transport, and shopping assistance.
- Manage client information in CRM & CMS, ensuring all details and case notes are up-to-date.
- Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team as required.
Your day will be spent on the road completing scheduled visits, interacting one-on-one with clients within their homes, supporting with daily activities, providing care in accordance with their individualized care plan, and supporting with community-based recreational and social activities.

- Experience with technology, including smartphones, tablets, and laptops.
- Confidence to satisfy relevant probity checks required by legislation or policies.
