Senior Physiotherapist or Occupational Therapist

We are seeking a highly skilled and experienced Physiotherapist or Occupational Therapist to join our Community Rapid Triage Team as a Senior Clinician. As a key member of our team, you will be responsible for assessing clients following their discharge from an Eastern Health hospital who have been referred to the Community Rehabilitation Program.
Key Responsibilities- Assess clients and develop treatment plans to support their recovery and independence
- Work collaboratively with other healthcare professionals to provide holistic care and support
- Recognize and escalate deteriorating patients to medical staff to prevent avoidable readmissions to hospital
- Participate in quality improvement initiatives to enhance patient outcomes and service delivery
- Demonstrated expertise in managing complex and vulnerable populations
- High-level clinical reasoning and advanced casework skills
- Ability to work autonomously and collaboratively as part of a multidisciplinary team
