Job Title: Transitional Care Nurse

Key Responsibilities:
  • Deliver nursing care to individuals requiring support with daily living activities.

  • Work collaboratively with multidisciplinary teams to ensure seamless transitions between care settings.

Selection Criteria:
  • Current registration as a Registered Nurse with the Australian Health Practitioners Regulation Agency.
  • Demonstrated experience in providing high-quality, person-centered care in a transitional care setting.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients and their families.
