Clinical Case Manager

Role DetailsThis is a permanent part-time position, working 26 hours per week. The successful candidate will be responsible for delivering health services to clients in Hobart, working collaboratively with other health professionals to assess, plan, and implement restorative or reablement activities.
Key Responsibilities- Conduct health assessments and develop personalized care plans for clients
- Work collaboratively with clients, carers, and families to promote health literacy and empowerment
- Coordinate and deliver Short-Term Restorative Care programs in Hobart
- Monitor client health outcomes and adjust care plans as needed
- Build and maintain relationships with local health professionals and services to ensure high-quality care
- Degree qualification in nursing or allied health
- Registration with relevant professional body or Australian Health Practitioner Regulation Agency (AHPRA)
- 3+ years of clinical or health management experience in Nursing or Allied Health
- Experience within a health or community aged care setting
- Ongoing commitment to Continued Professional Development and Education
- Demonstrated understanding of community aged care guidelines and the principles of primary health care and health promotion
- Demonstrated understanding of evidenced-based care
- Ability to conduct Case Management and Health assessments
- Strong interpersonal skills, problem-solving, analytical, and reporting skills
- Ability to work as part of a wider team and autonomously in a remote model of working
- Ability to organize work in a systematic, timely, and accurate manner
- Well-developed IT skills and demonstrated competence in delivering services via virtual health technologies and a willingness to learn and adopt new technologies
- Current driver's license and ability to travel within the region on a regular basis
- Satisfactory Police Check and Current CPR
