Job Title: Compassionate Care Coordinator

Description:

We are seeking a skilled and compassionate Clinical Care Partner to join our team. As a key member of our care coordination team, you will play a vital role in ensuring that our clients receive high-quality, person-centred care.

About the Role:

This is an exciting opportunity to work with a dedicated team of professionals who share your passion for delivering exceptional care to our community. As a Clinical Care Partner, you will be responsible for coordinating and overseeing complex clinical and care needs through assessment, care planning, and coordination of services.

You will work closely with clients, their families, and multidisciplinary teams to develop and review individualized care plans that cater to each client's unique needs and preferences.

Key Responsibilities:

  • Develop and review comprehensive care plans in collaboration with clients, families, and healthcare teams
  • Coordinate and facilitate care activities, including assessments, interventions, and service referrals
  • Liaise with clinical teams to monitor and respond to changing client needs
  • Continuously evaluate and improve care outcomes, identifying opportunities for growth and development
  • Collaborate with internal stakeholders to ensure seamless care delivery and effective communication

Requirements:

  • Registered Nurse or Allied Health professional with current AHPRA registration and unlimited work entitlements
  • Demonstrated understanding of client-centred care principles and practices
  • Excellent communication and interpersonal skills, with ability to build strong relationships with clients, families, and colleagues
  • Ability to work autonomously, prioritizing tasks and managing time effectively in a dynamic environment
  • Current Australian Driver's Licence and access to a roadworthy vehicle
