Job Description

We are seeking a dedicated and passionate Case Manager to join our team in Charlestown, NSW. This full-time role involves working closely with children and young people in foster care, providing support and guidance to help them achieve their goals.

The successful candidate will have experience in case management and/or working with families or children and young people. They will be able to promote the safety, welfare and wellbeing of children and young people through effective case work and direct support.

  • Promote the safety, welfare and wellbeing of children and young people through case work and support.
  • Maintain client records and case management systems according to policies, procedures, standards and legislation.
  • Attend supervision meetings and ongoing training as required by management.
  • Advocate for clients and promote their best interests; provide support to a child or young person in care and to their carer families (e.g., home visits, trauma-informed care education, behaviour support strategies, referrals to therapeutic services).
