About This Role

This is an exciting opportunity to work as a physiotherapist in a home and community services setting. As a physiotherapist, you will be responsible for conducting comprehensive assessments of clients' physical and functional abilities to understand their needs.

You will develop and deliver individualised or group treatment plans based on clients' needs, goals, and preferences. Your expertise will be utilised to implement evidence-based physiotherapy interventions to address mobility issues, pain management and other age-related conditions.

Key responsibilities include regularly assessing and reporting on the physical and psychosocial capabilities of clients to the care management team. You will also maintain, prepare and support precise records, exercise sheets, assessment reports and progress notes that accurately reflect clients' physical function.

In addition, you will make recommendations regarding exercise programs, mobility equipment, safety at home and refer to other disciplines as required. You will uphold ethical and professional standards in accordance with regulatory guidelines and collaborate with interdisciplinary teams to provide holistic care.

You will educate clients and families on preventive measures and exercises to improve and maintain mobility. Participating in team meetings, case conferences and training sessions to contribute to continuous improvement of care delivery is also essential.

To succeed in this role, you will need to possess a Bachelor or Post Graduate Degree in Physiotherapy and current AHPRA registration. Excellent interpersonal, verbal, and written communication skills are also necessary.

Able to manage all aspects of compliance, demonstrated experience with multidisciplinary teamwork and effective time management to ensure patient intervention, reports and outcomes are achieved in a timely manner are key requirements.
