Physiotherapist - Home and Community Services

Location – travel across the central coast (primarily northern c.coast)

Permanent Part Time (3 days) | Pay rate – up to $54.50 per hour (based on experience) + superannuation

Responsibilities
  • Conduct comprehensive assessments of clients' physical and functional abilities to understand their needs
  • Develop and deliver individualised or group treatment plans based on clients' needs, goals, and preferences
  • Implement evidence-based physiotherapy interventions to address mobility issues, pain management and other age-related conditions
  • Regularly assess and report on the physical and psychosocial capabilities of clients to the care management team
  • Maintain, prepare and support precise records, exercise sheets, assessment reports and progress notes that accurately reflect clients' physical function
  • Make recommendations regarding exercise programs, mobility equipment, safety at home and refer to other disciplines as required
  • Uphold ethical and professional standards in accordance with regulatory guidelines
  • Collaborate with interdisciplinary teams to provide holistic care
  • Educate clients and families on preventive measures and exercises to improve and maintain mobility
  • Participate in team meetings, case conferences and training sessions to contribute to continuous improvement of care delivery
  • Engage with team members, clients and families in a positive and respectful manner, welcoming a culturally diverse team
  • Build relationships with clients and their families, understanding challenges in the aged care environment
To be successful in this role you'll need
  • Bachelor or Post Graduate Degree in Physiotherapy and current AHPRA registration
  • Excellent interpersonal, verbal, and written communication skills
  • Ability to manage all aspects of compliance
  • Demonstrated experience with multidisciplinary teamwork
  • Effective time management to ensure patient intervention, reports and outcomes are achieved in a timely manner
  • Ability to prioritise and organise own work/caseload
  • Demonstrated computer literacy
  • Demonstrated knowledge and experience of health, aged and/or disability services
  • Current Driver's licence and registered and insured motor vehicle
